  • Best not describe an 'unexplained' incident as an accident, which implies unintentional.
  • No sure if he was meant to be repairing it; considered calling it maintenance, but wasn't strictly sure of that either, so went for the generic "working on" though avoiding use of the same phrase would have been nice.
